Who We Are:
The VNA of Manchester and Southern New Hampshire provides 24/7 in-home support and services to patients in Manchester and the surrounding communities. We are committed to helping our patients navigate the challenges they face at home while recovering from surgery or coping with life-limiting illnesses or physical disabilities.
Our nurse-led interdisciplinary team includes skilled nurses and nurses’ aides, physical, occupational, and speech therapists, and social workers. The VNA care team provides individualized care across the continuum, bringing peace of mind to patients and their loved ones while providing professional healthcare services in the comfort of home.

Home Health is a specialized setting of care that’s incomparable to any other, providing a unique insight into those factors that can influence how patients manage their health conditions at home. We are highly skilled nurses– discovering home safety deficits, nutrition and hydration concerns, potential medication errors, and assessing the impact of social determinants of health.
The VNA care team works together with the patient, their caregivers, and ambulatory providers to deliver the right combination of services to facilitate great outcomes. Whether our patient is recovering from a single acute event or living with a new or chronic illness, our nurses provide education, safety recommendations, and hands-on skilled interventions to help reduce future hospitalizations.

What You’ll Do:
- Comprehensive patient assessment
- Medication reconciliation and education
- Disease process education
- Wound and ostomy care
- Home infusion therapy
- Skilled nursing interventions such as pleural drainage, urinary catheter management, etc.

Work Schedule: Per Diem- Every other weekend commitment, holiday and on call.
